The roles and responsibilities include but are not limited to:Assist in conducting in-home client assessments and reassessments and offer creative care solution that best meets the client’s needs.Play a significant role in ongoing education and learning with caregivers including reviews, delegations and coaching.Lead caregiver education programs including content and delivery.Work directly with clients to assist in navigation of a complex health care system.Develop the most appropriate care plan based on the client’s needs.Play an important role in identifying caregivers that would best suit the client.Working closely with the Care Coordinators team as a key resource.Ongoing development and improvement of best practices.

MINIMUM REQUIREMENTS:Requirements:Registered Nurse with current College of Nurses of Ontario licence.Driver’s license and willingness to use your own vehicle.Previous experience providing support in the capacity as an RN.Excellent written and verbal communication skills.Preferred requirements:3-5 years of community health care experience.Management experience.Registered nurse.Bilingual.Customer service experience.Knowledge of all policies related to home health and community care.Knowledge of health and safety policies/procedures related to community care.Whims, WSIB.
