Supervisor of Health Navigation – The Esplanade

This is a Contract position in Toronto, ON posted February 2, 2021.

The Esplanade Hotel, located at Yonge and The Esplanade in Downtown Toronto, is the location of a nearly 250-room program for adults experiencing homelessness. Homes First is committed to the development of on-site support in order to best serve the residents.

The Supervisor of Health Navigation ensures that clients of all backgrounds are equipped with the necessary information and supports to engage with community systems in Toronto, including but not limited to health care and government programs. This position also assists clients with low language skills, mental and physical health needs, and addictions in accessing specialized care that is often beyond the scope of what is normally provided by government services. This position will work collaboratively with the Site Manager and ICM team to meet client needs. This position is also required to make follow up phone calls to clients and effectively track them if needed while admitted to hospital or other programs. Evening and weekend work may be required on occasion.


  • Work with staff to identify and assist and provide expertise for clients in need of assistance in all community contexts including but not limited to healthcare, addiction services, government programming, community supports, etc. with the goal of empowering clients as much as possible;
  • Supervise a unionized workforce within the context of the Collective Agreement;
  • Manage employee performance and perform necessary administrative duties such as vacation planning, timekeeping, performance reviews, etc.;
  • Implement and maintain a system for collecting relevant statistics needed for evaluations report and funding proposals;
  • Ensure effective communications with other departments within Homes First Society and external agencies;
  • Manage health activities with clients;
  • Coordinate with clients and staff to ensure clients’ needs are being met;
  • Develop and assist staff in execution of complex case plans for clients with mental health needs with a particular focus on those in crisis, including if necessary interventions involving the Mental Health Act, MCIT, and Form 2s;
  • Liaison with hospitals while clients are admitted including collaborating in discharge planning;
  • Deliver frequent information and orientation sessions regarding local hospitals, health clinics and other access points to care;
  • Orient clients to prescriptions, as required;
  • Foster strong relationships with healthcare providers, pharmacies, dentists and opticians who support residents either on-site or in the community
  • Develop meaningful resources in connection with client’s case plan regarding health and well-being, including mental health;
  • Support/direct the ICM Team and other staff with making follow up phone calls to clients;
  • Work with staff to ensure current client records are maintained;
  • Attend staff meetings;
  • Be able to work occasional weekends and/or evenings, when required;
  • Be willing and able to carry 24-hour emergency pager on rotation;
  • Working in an environment where there may be smoke is required;
  • Other responsibilities, as required.


  • A degree in a relevant discipline or equivalent combination of education and experience;
  • At least three years’ significant experience with people who are/have experienced homelessness, use drugs, racialized populations, Indigenous Peoples, people who engage in sex work, have mental health issues, and have had involvement with the court/jail/prison systems;
  • Ability to work effectively in a collaborative team environment;
  • Excellent communication, organization and time management skills;
  • Supervisory experience in a unionized setting an asset;
  • Excellent interpersonal, conflict resolution and organization skills;
  • Certificates in First Aid, CPR, Crisis Prevention and Intervention an asset;
  • The successful candidate will be required to work where smoking is present.

Working in an environment where there may be smoke is required.
