Job Summary:

The CC2H/CDM Therapist in eHomecare is responsible to provide therapy services to individuals in the community within the framework of VON policies, procedures, standards, and quality and risk management.

Key Responsibilities:

  • Assesses and reviews patient’s outcomes, evaluate patient progress, re-evaluate patient goals and provides the appropriate care and/or direction of client care.
  • Demonstrates knowledge, skills and attitudes as outlined in the Therapy Standards and guidelines for an evidence-based practice, in order to assess, plan, evaluate progress (for potential discharge) and document patient plan of care within a patient centered approach to therapy goals.
  • Collaborates with Health Care Technician (HCT)  to obtain patient information about their health, associated history, previous health interventions and outcomes as appropriate.
  • Collects assessment information relevant to client’s needs and therapy practice.
  • Analyzes assessment findings to establish a therapy diagnosis and prognosis.
  • Develops and recommends a plan of care with the patient/family and health care team, provides direction of plan of care to the HCT as appropriate.
  • Monitors patient outcomes and progression towards therapy goals.
  • Modifies the patient’s care plan as required based on patient outcomes. Any changes to the care plan must be developed and modified by the therapist for safety and liability.
  • Documents patient outcomes and current care plan in the electronic patient chart for the interdisciplinary team.
  • Communicates with interdisciplinary team as required regarding patient outcomes and status changes (e.g. most responsible physician).
  • Provides ongoing direction to the HCT through the use of a mobile device as required (i.e. phone, tablet, OTN)
  • Completes expectations related to logging in/out of the innovative integrated technologies for directing the HCT during tethered care.
  • Utilizes verbal and non-verbal communication skills effectively with the patient/family as required.
  • Demonstrates professional code of conduct / communication with all members of the health team as per VON guidelines and policy.

External and Internal Relationships:

  • Works collaboratively with the VON eHomecare regional and interdisciplinary team regarding patient outcomes and status changes.
  • Works collaboratively with the LHIN and hospital partners to ensure a safe transition to community and patient goals are met.

Education, Designations and Experience:

  • Masters in applicable Therapy stream (Occupational/Physiotherapy/Speech-Language Pathology).
  • Valid registration with the associated college of Therapy.
  • Member with the respective Therapist association is preferred.
  • Minimum of 2 years’ experience in a health care environment preferred (i.e. long term care, acute medicine, rehab unit, rehab clinic or community visiting program).
  • Recent demonstrated knowledge and experience in stroke best practices and musculoskeletal (i.e. arthroplasty) best practices is preferred.
  • Knowledge of chronic disease management and strategies to support patients living in community.
  • Demonstrates knowledge of community resources preferred.
  • Demonstrates knowledge of standards for the supervision of support personnel (Therapy Assistant) in their practice.

Skill Requirements:

  • Ability to utilize computer software program(s) on a hand held device (i.e. phone or tablet).
  • Demonstrates proficiency in the use of technology for documentation of patient outcomes.
  • Ability to supervise and monitor the practice of Therapy Assistants according to the standards of their registered governing association.
  • Ability to communicate well, both written and verbal.
  • Ability to work interdependently and collaboratively with a multidisciplinary team.
  • Ability to adjust to working in different social and economic environments.
  • Ability to respond to changes in patient caseloads and care needs.


  • Must have personal vehicle and possess both a current driver’s license and proof of vehicle insurance
  • Willingness to travel regularly during the day as well to support sites and attend meetings.
  • A current and original copy of a satisfactory Criminal Records Check is required.
  • A Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
  • Ability to speak French is an asset
  • Must be able to wear Personal Protective Equipment (PPE)
