This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.

Responsibilities Nova Scotia Health is leading a virtual healthcare platform to support the health and health care of Nova Scotians.

Reporting to the Health Services Manager of Primary Health Care (PHC) or designate, the Family Practice Nurse provides comprehensive and coordinated care to patients and their families as a member of the Virtual Care primary care team.

The Registered Nurse partners with and complements other members of the health care team for the achievement of person and family-centered care, screening and health promotion, early detection of illness and disease, chronic disease management, continuity and coordination of care and enhanced access to programs, services and community resources.

The virtual primary care team works together to develop and deliver services to the entire population in the community clusters who do not have a primary care provider, including outreach to remote areas and services to meet the needs of priority populations.This position requires providing care both virtually and in person and may require work at multiple sites and travel between various community locations throughout the area.

The RN independently provides safe, competent, compassionate, ethical nursing care to stable, predictable and unpredictable populations, and ensures the goals and needs of the patients and families are prioritized and individualized.

The RN works collaboratively with the team to promote therapeutic relationships with patients, efficient and effective teamwork, and the optimal scope of practice for all team members through assessment, planning, implementation and evaluation of care plans for patients, families and communities.

The RN also contributes to population health using assessment, analytical and advocacy skills.

Nursing services include advocacy, assessment and continuity of care, health promotion and disease prevention, patient education, liaison to community resources; supportive counseling and clinic coordination.

Registered Nurses in Primary Health Care adhere to the CRNNS Standards for Nursing Practice and the Code of Ethics.

The FPN ensures expected behaviors, the leadership philosophy, organizational health, population health, ethics, safety, quality, partnerships and interdisciplinary collaboration are integrated within the support that is provided by the work that they do and through the messages they deliver.

Qualifications Graduate of an accredited school of nursing, Bachelor degree in Nursing preferred Currently registered as a RN, and in good standing with College of Registered Nurses of Nova Scotia (CRNNS), or eligible Minimum two (2) years’ experience as a Registered Nurse required Successful completion of the Family Practice Nursing Program or equivalent recognized program, or currently enrolled in the program required Recent experience in a primary care collaborative team setting or relevant practice area preferred Membership with the Canadian Family Practice Nurses Association (CFPNA) and Family Practice Nurses Association of Nova Scotia (FPNANS) an asset Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning), care planning, case management, evaluation of client competence in self-care and other relation outcomes coupled with the provision of nursing care in a Primary Health Care environment Demonstrated ability to communicate effectively with patients, families, public, community partners, primary care team members and other professionals and agencies using verbal, written and computer communication means Demonstrated critical thinking skills within scope of practice including clinical judgment and problem solving skills and the ability to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively Demonstrated leadership capabilities and strong ethical standards and a commitment to professional development and life-long learning and ability to promote client-focused care that demonstrates care for and with clients and significant others, sensitive to diverse cultures and preferences, client advocacy and social justice concerns Strong understanding of health promotion, population health, and the determinants of health theory and practice Ability to transmit information grounded in behavior change and principals of self-management intended to instruct clients and others about topics essential to health care and well-being and demonstrated ability in applying new and innovative technologies and strategies for education and patient counseling along with experience facilitating education programs and group meetings Ability to manage time and resources, implementing activities to promote cooperation among relevant others, collaboration across disciplines and related activities Ability to promote staff morale, cooperation, assertiveness and risk-taking, creative planning for change and innovations, implementation of NSHA policies or other protocols, and ongoing professional development of self Ability to use factual information, prior learning and basic principles and procedures to support decisions and actions with relevant research-based evidence and integrate best practice from nursing and health-related disciplines and the humanities, arts and sciences disciplines into professional practice along with commitment to continuous learning for self, interprofessional team and supportive of interprofessional health discipline students through formal/informal mentoring opportunities Hours of Work Long Assignment, Full-time position; 75 hours biweekly Beginning May 10, 2021 with an anticipated end date of May 10, 2022 Dates are approximate and subject to change Shifts may include evenings, weekends, and holidays Salary Information Nursing NSNU Nursing (updated 2021 rates) Registered Nurse: $35.2108
