Carlington Community Health Centre: Counsellor Intake Outreach Worker

This is a Full-time position in Ottawa, ON posted September 11, 2020.

The Counsellor Intake provides crisis intervention, system navigation, harm reduction, case management, outreach, information, support, and advocacy, referral, and short-term supportive counselling to clients of Carlington and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.Program-Related ActivitiesEngages in short-term assessment, referral, and advocacy activities with clients referred by Carlington staff and other area workers.Acts as a resource to and advocate for individuals, groups, and the community.Provides risk assessment, personal and practical support, information, referral and advocacy with individuals.

Assesses client status by gaining an understanding of social determinants of health.Acts as a client advocate on individual and systemic issues.Provide crisis intervention services both over the telephone and in person.Provides harm reduction services to clients at the centre and on an outreach basis as required.

Acts as a liaison for the harm reduction workers in the community and community partner agency.Contributes to chart documentation, participates in chart reviews and case conferences, reflective practice meetings/discussions and makes referrals.Participates in the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services for individuals, families and the community.Acts as a consultant on resources for other service providers in the Centre and in the community.Liaises and collaborates with staff, teams and other community service providers as appropriate to provide service and make referrals.Assists clients and the community to build support networks.Ensures accountability by providing statistics and reports of activities, as required, and keeping client records, as required, within the safeguard of confidentiality.Participates in the selection, orientation and supervision of volunteers as applies to program related activities.Participates in the selection, orientation, training and supervision of students in practicum placements as required.EducationBachelor’s degree from a recognized university in a relevant discipline or equivalent combination of some post-secondary education and experience in counselling.Professional experienceThree to five years full-time relevant experience in a community setting.Experience in providing community-based mental health and counselling services including crisis intervention, short-term supportive counselling, harm reduction, group work, referral, and advocacy.Experience and interest in working with marginalized and diverse clientele, such as sex-trade workers, and low-income, high-needs, newcomer, and a variety of multicultural communities.Experience in program development, implementation, monitoring, and evaluation.Experience in a non-profit, community-based organization.Key CompetenciesThorough knowledge of and proficiency in current assessment and intervention techniques.Thorough knowledge and understanding of community resources, relevant city service-provider networks, community-based health care, and income-maintenance legislation and services, as well as issues affecting women and people affected by mental illness.

Knowledge of the Carlington client-service area an asset.Acquired knowledge in the principles of social prescribing.Excellent knowledge of and experience with a harm reduction approach.Experience working with women engaged in the sex trade as well as knowledge of relevant support systems and resources.Excellent interpersonal and communication skills, oral and written.Excellent time management and organization skills.Demonstrated ability to work effectively in a crisis situation.Demonstrated ability to work in a multi-disciplinary team environment.Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client services setting.Linguistic ProfileThis position falls within Carlington CHC’s designation under the French Language Services Act of Ontario.Fluency in English is required.Fluency in French is required at Level (Advanced +) for oral expression/comprehension and Level (Intermediate +) for written expression.Fluency in another language relevant to our client population is an asset.OtherAvailability to work evenings and weekends if required.Valid drivers licence and access to a vehicle in order to do outreach work.Food Handler Certification.First Aid/CPR certification.About CarlingtonCarlington Community Health Centre is a non-profit organization that puts today’s most pressing health care challenges at the heart of their service delivery.
