We also take time to recognize each other and have fun!We are currently seeking Social Workers to join our Assertive Community Treatment Team (ACTT).The ACT Team is a client centered, recovery oriented specialized mental health service delivery model that provides supports to clients living in the community.

These clients have serious and persistent mental illness and significant functional impairments.

The Team is made up of Nurses, Addiction counsellors, social work, Recreation Therapy, Occupational Therapy, Peer Support and Psychiatry.

Using the principles of client-centered care within the context of a multi-disciplinary team approach the nurse is responsible for conducting comprehensive bio-psychosocial assessments, mental status and physical health assessments; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing intensive treatment, rehabilitation, and community support services.

HOURS OF WORKThis relief position requires working a weekend rotation schedule (1 in 6 weekends).

As a Relief employee you may also be contacted for additional shifts which include day, afternoon, evenings.Required Qualifications:Bachelor or Master’s Degree in Social Work from an accredited university.Registration with the Ontario College of Social Workers and Social Service Workers.Three to five years’ experience in a community mental health setting working with adults recovering from serious mental illness.Certification in CPR, non-violent crisis intervention and in suicide risk assessment preferred.Experience in a community mental health setting working with adults recovering from serious mental health illness.

Preferred Qualifications:Knowledge of mental health resources within the Waterloo Region, considered an asset.Certification in CPR, non-violent crisis intervention and in suicide risk assessment preferred.Demonstrated skill in applying the principles of PSR, Recovery and case management.Ability to work within the agency’s mission and values.Working knowledge of the Mental Health Act and other relevant legislation.Willing to travel extensively within the catchment area and to work evenings and weekends.Must have a valid driver’s license, relevant insurance and use of a reliable vehicle.Proficient in PC skills (MS Office Suite).Provide a satisfactory police vulnerable sector check.
